Here's what we'll cover

Not long ago, GLP-1 treatment for weight loss could cost $1,000 or more a month without insurance. The only way most people found a $150 price was through compounded medications. That has changed. In 2026, FDA approved brand name GLP-1 pills start at about $149 a month through manufacturer self pay programs.

Here is what $150 a month actually gets you now, what costs rise over time, and how to compare options safely.

Why Prices Dropped

Cost has been a major barrier. In a late 2025 KFF poll, 56% of GLP-1 users said the drugs were difficult to afford, and 14% said they had stopped taking them because of cost. Since then, manufacturers have expanded direct to patient self pay programs, two GLP-1 pills have launched, and retail pharmacies now list the same manufacturer prices.

What $150 a Month Buys in 2026

As of October 2026, manufacturer self pay prices compiled by Kymata Health were:

MedicationFormSelf pay monthly priceFDA approval
Wegovy pillDaily tablet$149 (1.5 mg), $199 (4 mg), $299 (9 mg or 25 mg)Weight management and cardiovascular risk reduction in adults
FoundayoDaily tablet$149 to start, about $299 at maintenanceWeight management in adults
Wegovy penWeekly injection$199 starter doses, $349 (1 to 2.4 mg), $399 (7.2 mg)Weight management, cardiovascular risk reduction, MASH
ZepboundWeekly injection$299 (2.5 mg), $399 (5 mg), $449 (7.5 to 15 mg)Weight management and sleep apnea

So the $150 headline is real, but it usually reflects the starting dose. As you move up to maintenance doses, most self pay prices rise to roughly $299 to $449 a month.

The Two Pills Compared

The Wegovy pill is oral semaglutide. It produced 13.6% average weight loss at 64 weeks in the OASIS 4 trial, but must be taken on an empty stomach with up to 4 ounces of water and a 30 minute wait before eating. Foundayo (orforglipron) can be taken with or without food. In its trials, the highest dose produced about 11% average weight loss at 72 weeks.

Compounded GLP-1 Programs Now

Many earlier $150 programs relied on compounded semaglutide or tirzepatide. The shortage that allowed widespread compounding ended for tirzepatide in December 2024 and for semaglutide in February 2025. In an April 2026 statement summarized by Polsinelli, the FDA reminded compounders that neither drug appears on its 503B bulks list or its drug shortage list, and that pharmacies generally may not make essentially copies of approved drugs unless a prescriber documents a patient specific clinical need.

Compounded GLP-1s are not FDA approved. As of May 31, 2026, the FDA had received 990 adverse event reports linked to compounded semaglutide and more than 730 linked to compounded tirzepatide, some tied to dosing errors. The FDA also warns about counterfeit and illegally marketed products sold online.

If a program still offers compounded medication, ask why it is clinically necessary for you, which licensed pharmacy makes it, and what happens if supply stops.

Questions to Ask Any Low Cost Program

  • Is the medication FDA approved brand name, or compounded?
  • What will I pay at each dose as I titrate up, not just at the start?
  • Are visits, follow ups, and messaging included or billed separately?
  • Who reviews my health history, and is lab work required?
  • How are side effects and dose changes handled between visits?

Other Ways to Lower Cost

  • Commercial insurance with a manufacturer savings card can bring Wegovy or Zepbound to as little as $25 a month for eligible patients.
  • Eligible Medicare Part D enrollees can get Wegovy, Zepbound, or Foundayo for $50 a month through the Medicare GLP-1 Bridge until December 31, 2027.
  • A prior authorization or appeal may unlock coverage if you meet your plan's criteria.
  • Check GLP-1 coupons and savings programs for current offers before you buy.

Who These Programs Suit Best

Low cost self pay programs work well for people without coverage who are comfortable with telehealth care and can plan for higher maintenance dose prices. People with complex conditions such as kidney disease, a history of pancreatitis, or thyroid cancer risk factors should seek closer specialist oversight.

Frequently Asked Questions

Can I really get a GLP-1 for $150 a month?

Yes, at the starting dose. As of October 2026, the Wegovy pill was $149 for 1.5 mg and Foundayo started at $149 through manufacturer self pay programs. Maintenance doses cost more.

How much do GLP-1s cost at maintenance doses?

Typical self pay prices are about $299 for the Wegovy pill at 9 or 25 mg, about $299 for Foundayo, $349 for Wegovy pens at 1 to 2.4 mg, and $299 to $449 for Zepbound.

Is compounded semaglutide still available?

Widespread compounding is restricted because semaglutide and tirzepatide are no longer on the FDA shortage list. Compounding is generally limited to documented patient specific clinical needs.

Are compounded GLP-1s safe?

They are not FDA approved. As of May 31, 2026, the FDA had received 990 adverse event reports for compounded semaglutide and more than 730 for compounded tirzepatide, some linked to dosing errors.

Which is cheaper, the Wegovy pill or Foundayo?

Both start at about $149 a month and reach about $299 at maintenance. The Wegovy pill requires an empty stomach routine, while Foundayo can be taken with or without food.

Does Medicare cover GLP-1s for weight loss?

Through the Medicare GLP-1 Bridge, eligible Part D enrollees can get Wegovy, Zepbound, or Foundayo for $50 a month until December 31, 2027.

The Bottom Line

Affordable GLP-1 treatment is real in 2026, and it no longer depends on compounded drugs. Brand name GLP-1 pills start at about $149 a month through manufacturer self pay programs, though maintenance doses usually cost about $299 to $449 a month.

Because weight often returns after stopping, as the STEP 1 extension showed, budget for long term use, not a three month trial. Compare total costs at every dose, confirm whether a product is FDA approved, and choose a program with real clinical oversight. To get started, compare vetted telehealth providers. Always consult your physician before starting, switching, or stopping any medication.
