Here's what we'll cover

If you are taking a GLP-1 medication and seeing results, you may wonder whether this is as good as obesity treatment gets. Probably not. Researchers are building on the GLP-1 foundation with drugs that target several hormone pathways at once, and the latest Phase 3 results are striking.

This is not about replacing semaglutide or tirzepatide. It is about what comes next for patients who need more, and what the evolving science means for your decisions now.

Why GLP-1 Drugs Alone May Not Be Enough for Everyone

Semaglutide 2.4 mg, sold as Wegovy, produced about 15% average weight loss over 68 weeks in its pivotal trial. That was a major advance. But for someone who needs to lose 100 pounds or more, 15% may only be a starting point.

Obesity is a complex disease. It involves a network of signals that regulate hunger, fullness, energy storage, and metabolism. Targeting only one part of that network has limits.

The Plateau Problem

Many patients on GLP-1 therapy hit a plateau after several months. Appetite suppression continues, but weight loss slows or stops before reaching a meaningful target. That is not a personal failure. It reflects how strongly the body defends its weight. Combination therapy is designed to push against that defense from several directions.

What Is Combination Therapy, Exactly?

Combination therapy means using two or more mechanisms together, either packed into one molecule or given as separate drugs, to amplify weight loss beyond what one agent can achieve.

The idea is familiar in medicine. HIV, cancer, and blood pressure treatment all rely on combinations. Obesity medicine came to it later, partly because of regulatory caution after the fen phen era.

Single Molecule vs. Coadministration

Multi agonist molecules pack several hormone actions into one injection. Tirzepatide, approved as Mounjaro and Zepbound, activates both GLP-1 and GIP receptors. At its highest dose it produced about 21% average weight loss over 72 weeks in SURMOUNT 1.

Coadministration combines two separate drugs with different mechanisms, sometimes in a single pen. This opens the door to pairing a GLP-1 with an entirely different drug class.

Triple Agonists: Retatrutide's Phase 3 Results

The most discussed next step is the triple agonist, a molecule that activates GLP-1, GIP, and glucagon receptors together. Eli Lilly's retatrutide is furthest along.

In its Phase 2 trial published in 2023, retatrutide produced about 24% average weight loss at 48 weeks on the highest dose. Phase 3 has now confirmed the signal. In TRIUMPH 1, Lilly reported that participants on 12 mg lost an average of 28.3% of body weight over 80 weeks, and 45.3% lost at least 30%.

In July 2026, Lilly announced positive results from two more Phase 3 trials in people with type 2 diabetes and in people with severe obesity and heart disease. The company says it plans to submit retatrutide to the FDA in the first quarter of 2027. It is not approved and is legally available only inside Lilly's clinical trials.

Why Glucagon? Is That Not the Opposite?

Glucagon raises blood sugar, which sounds counterproductive. In these molecules, though, the glucagon component appears to increase energy expenditure and fat burning in the liver, while the GLP-1 component offsets the blood sugar effect. The result appears additive rather than redundant.

CagriSema: The Combination Closest to an FDA Decision

CagriSema is Novo Nordisk's once weekly injection combining cagrilintide, an amylin analog, with semaglutide. Amylin is released alongside insulin and signals fullness through a different pathway than GLP-1, so the two may work together rather than overlap.

What Phase 3 Has Shown

In the REDEFINE 1 trial of adults without diabetes, CagriSema produced 20.4% average weight loss at 68 weeks versus 3.0% with placebo. Among people who stayed on treatment the figure was about 23%. Side effects were mainly gastrointestinal, similar to semaglutide alone.

The picture is not uniformly positive. In a head to head trial reported in February 2026, CagriSema did not meet its goal of matching tirzepatide 15 mg, with 23.0% versus 25.5% weight loss at 84 weeks. Novo Nordisk is now testing higher doses.

Novo Nordisk filed CagriSema with the FDA on December 18, 2025. The company expects a decision in the fourth quarter of 2026, though no official decision date has been published. Until then it is available only in clinical trials.

What This Means for the Cost of Obesity Care

New combination drugs will almost certainly launch at premium prices. Insurance coverage for new obesity medications usually lags approval, and early access tends to favor patients with documented high risk conditions.

Current options have become more affordable. Self pay Wegovy costs $349 a month through NovoCare, and Zepbound vials cost $299 to $449 a month through LillyDirect.

Cost Strategies Worth Knowing Now

  • Manufacturer savings and self pay programs from Eli Lilly and Novo Nordisk can sharply reduce out of pocket costs.
  • Telehealth providers often bundle visits and medication at competitive prices.
  • Compounded semaglutide and tirzepatide have narrowed sharply under FDA enforcement, so do not build a long term plan around them.

Questions to Ask Your Doctor Now

1. Am I on the most effective option available for my situation?

If you are plateauing on semaglutide, tirzepatide may offer more. Your provider should evaluate whether switching makes sense.

2. Is there a clinical trial I might qualify for?

ClinicalTrials.gov lists ongoing combination therapy trials. Participation can provide access to new drugs at no cost, with close monitoring.

3. Should I add structured behavioral support?

The trials with the best results include lifestyle components. Medication works best with them.

4. What is the plan if I plateau?

Ask now, not after six months of stalled progress.

Frequently Asked Questions

What is combination therapy for obesity?

Combination therapy targets two or more hormone pathways at once, either in a single multi agonist molecule or by giving separate drugs together. The goal is more weight loss than a single agent can deliver by working against the body's weight defense from several directions.

Is tirzepatide already a combination therapy?

Yes. Tirzepatide, sold as Mounjaro and Zepbound, activates both GLP-1 and GIP receptors. At its highest dose it produced about 21% average weight loss over 72 weeks in the SURMOUNT 1 trial.

What is retatrutide and when will it be available?

Retatrutide is an Eli Lilly triple agonist targeting GLP-1, GIP, and glucagon receptors. In the Phase 3 TRIUMPH 1 trial it produced 28.3% average weight loss at 12 mg over 80 weeks. Lilly plans to submit it to the FDA in the first quarter of 2027, so approval would come later. It is not available outside clinical trials.

What is CagriSema and is it approved?

CagriSema combines cagrilintide, an amylin analog, with semaglutide in one weekly injection. It produced 20.4% average weight loss in the REDEFINE 1 trial. Novo Nordisk filed it with the FDA in December 2025 and expects a decision in late 2026. It is not yet approved.

Is CagriSema better than tirzepatide?

Not based on current data. In a head to head trial reported in February 2026, CagriSema produced 23.0% weight loss versus 25.5% for tirzepatide 15 mg and did not meet its goal of matching tirzepatide.

Can I take two weight loss drugs together right now?

Not outside a clinical trial or very close medical supervision. Most combinations are still being studied for safety. Talk to your doctor before combining any obesity medications.

The Bottom Line: GLP-1 Is the Foundation, Not the Ceiling

GLP-1 medications changed obesity medicine, but they are not the final word. Retatrutide's Phase 3 data and CagriSema's pending FDA decision show the field moving firmly toward multi mechanism treatment.

Still, the most effective combination available to you today is not a future drug. It is your current medication paired with the right provider, solid nutrition and behavioral support, and a clear plan for plateaus.

  • More powerful drugs on the horizon do not make your current treatment inadequate. Most people achieve meaningful results on semaglutide or tirzepatide.
  • Following the pipeline helps you ask better questions. It does not replace medical advice.
  • Cost will remain a barrier when new options launch, so learning the savings programs now is practical.
