Here's what we'll cover

On August 28, 2026, the FDA approved Mounjaro (tirzepatide) to lower the risk of major adverse cardiovascular events, meaning cardiovascular death, nonfatal heart attack, or nonfatal stroke, in adults with type 2 diabetes who are at high risk for these events.

Eli Lilly says Mounjaro is the first dual GIP and GLP-1 receptor agonist with this kind of indication. It is a meaningful addition to the label, but it is narrower than some early coverage suggested. It applies to people with type 2 diabetes, not to everyone with obesity and heart disease.

What the FDA Approval Actually Covers

The FDA approves drugs for specific indications backed by trial evidence. Mounjaro now has two:

  • Improving blood sugar control, along with diet and exercise, in adults and children 10 and older with type 2 diabetes.
  • Reducing the risk of major cardiovascular events in adults with type 2 diabetes who are at high cardiovascular risk.

Zepbound contains the same molecule and is approved for chronic weight management and obstructive sleep apnea. The new heart indication was granted to Mounjaro, the diabetes brand. If you have obesity and established heart disease but do not have type 2 diabetes, the GLP-1 with a cardiovascular label for you is Wegovy, which received that approval in 2024.

Who Counts as High Cardiovascular Risk?

The label refers broadly to adults at high risk, but the supporting trial enrolled people with type 2 diabetes and established atherosclerotic cardiovascular disease, such as a prior heart attack, stroke, or diagnosed coronary or peripheral artery disease. Your doctor can tell you whether your history fits.

The Evidence: SURPASS CVOT

SURPASS CVOT enrolled 13,299 adults aged 50 and older with type 2 diabetes and established cardiovascular disease. Unlike most heart outcomes trials, it did not use a placebo. It compared Mounjaro directly with Trulicity (dulaglutide), a GLP-1 drug that already carries its own cardiovascular indication.

Over a median of about four years, a major cardiovascular event occurred in 12.2% of people on tirzepatide and 13.1% on dulaglutide, a hazard ratio of 0.92. That met the goal of showing Mounjaro was noninferior. It did not prove Mounjaro was superior to Trulicity.

Because Trulicity had already been shown to reduce heart events, matching it is meaningful. Mounjaro also produced larger reductions in A1C and body weight. Gastrointestinal side effects were more common with Mounjaro, consistent with its known profile.

What This Approval Does Not Prove

It does not show that Mounjaro is better than Trulicity at preventing heart events, and it does not cover people without type 2 diabetes. It also does not mean Mounjaro replaces statins, blood pressure medications, or antiplatelet therapy. Drug interactions, kidney function, and other individual factors still matter.

How the GLP-1 Heart Labels Compare

MedicationActive IngredientCardiovascular IndicationWho It Applies ToKey Trial
MounjaroTirzepatideYes (August 2026)Adults with type 2 diabetes at high cardiovascular riskSURPASS CVOT (vs. Trulicity)
OzempicSemaglutideYesAdults with type 2 diabetes and established heart diseaseSUSTAIN 6 (vs. placebo)
WegovySemaglutide 2.4 mgYes (2024)Adults with obesity or overweight and established heart disease, with or without diabetesSELECT (vs. placebo)
ZepboundTirzepatideNoApproved for weight management and sleep apneaNot applicable

In the SELECT trial, Wegovy reduced major cardiovascular events by 20% versus placebo in people with obesity and heart disease who did not have diabetes. That remains the strongest heart outcome evidence for a GLP-1 in people without diabetes.

What This Means for Insurance Coverage

Most plans that cover Mounjaro already do so for type 2 diabetes, so this approval does not open a new coverage pathway the way a new obesity indication would. Its main effect is clinical. When a plan or prescriber chooses among diabetes medications for a patient with heart disease, Mounjaro can now be selected for proven cardiovascular benefit as well as blood sugar and weight effects.

Patient ProfileWhat Changes With This Approval
Type 2 diabetes with established heart diseaseMounjaro now has label support for heart risk reduction, which can strengthen prior authorization requests.
Type 2 diabetes without heart diseaseGlucose control indication unchanged; heart indication depends on whether your doctor judges you high risk.
Obesity and heart disease without diabetesNo change for Mounjaro. Wegovy is the GLP-1 with a cardiovascular indication for this group.
Obesity without diabetes or heart diseaseNo change. Zepbound or Wegovy remain the weight management options.

If you have type 2 diabetes and heart disease and were previously denied Mounjaro, ask your doctor to document your cardiovascular diagnosis and cite the new indication in any appeal.

What This Means for Your Cost

Mounjaro's list price is above $1,000 a month without insurance. Lilly's savings card can lower costs for eligible commercially insured patients with type 2 diabetes. Medicare Part D generally covers Mounjaro for type 2 diabetes. People with type 2 diabetes are not eligible for the separate Medicare GLP-1 Bridge, which is designed for weight management drugs.

Questions to Ask Your Doctor

  • Do I meet the criteria for Mounjaro's new cardiovascular indication?
  • Given my heart history, should I be on Mounjaro, Ozempic, Trulicity, or Wegovy?
  • If I am already on another GLP-1 with heart data, is there any reason to switch?
  • Will this approval help with my prior authorization or appeal?
  • Do any of my heart medications interact with tirzepatide?
  • How will we track whether my cardiovascular risk is improving?

Frequently Asked Questions

What is Mounjaro approved for now?

Mounjaro is approved to improve blood sugar in adults and children 10 and older with type 2 diabetes, and as of August 28, 2026, to reduce the risk of cardiovascular death, heart attack, and stroke in adults with type 2 diabetes who are at high cardiovascular risk.

Does the new approval apply to people without diabetes?

No. The cardiovascular indication is limited to adults with type 2 diabetes. For people with obesity or overweight and established heart disease who do not have diabetes, Wegovy is the GLP-1 medication with a cardiovascular risk reduction indication.

What trial was the approval based on?

SURPASS CVOT, which enrolled 13,299 adults with type 2 diabetes and established cardiovascular disease and compared Mounjaro with Trulicity. Major cardiovascular events occurred in 12.2% of people on Mounjaro and 13.1% on Trulicity, showing noninferiority. Superiority was not established.

Is Mounjaro the same as Zepbound?

Both contain tirzepatide. Mounjaro is the type 2 diabetes brand and now carries the cardiovascular indication. Zepbound is approved for chronic weight management and obstructive sleep apnea and does not have a cardiovascular indication.

How does Mounjaro compare to Ozempic and Wegovy for heart protection?

Ozempic has a cardiovascular indication for adults with type 2 diabetes and heart disease, and Wegovy has one for adults with obesity or overweight and heart disease regardless of diabetes. Mounjaro now has one for adults with type 2 diabetes at high risk. No trial has compared all three head to head for heart outcomes.

Will insurance cover Mounjaro for heart disease now?

Most plans that cover Mounjaro already do so for type 2 diabetes. The new indication can strengthen a prior authorization or appeal for patients with diabetes and heart disease, but it does not create coverage for people without diabetes.

The Bottom Line: What This Means for You

Mounjaro's cardiovascular approval is real and meaningful for adults with type 2 diabetes and heart disease. It gives patients and doctors an option that controls blood sugar, drives substantial weight loss, and has proven heart protection comparable to an established GLP-1.

It is not a heart indication for obesity alone. If you have heart disease without diabetes, Wegovy is the GLP-1 with cardiovascular approval for your situation. If you already take Mounjaro for diabetes and have heart disease, ask whether this changes your monitoring or coverage approach.

Consult your physician before starting, stopping, or changing any medication.
